I remember reading about the Orthodox Imperial Temple sometimes electing a new leader and 'the Land' (or Haelyn) just punching the pinball table to get a different result. I'm not sure how that would work in play.

I didn't say it was a smart choice, rolling the dice rarely is
And 'lost the game' is entirely subjective. You can have personal goals or realm goals which are achieved and still lose the character.
Rogr died but succeeded in his goals.
Meaghan succeeded in most of her goals before she got sent packing.
Makaria didn't succeed at much of anything (one turn regent).
Haelyn succeeded at most everything he sought to do.

This is the lose condition I'm talking about: 'NB: Having no designated heir, and having no children will result in the loss of your bloodline.'
'The loss of your bloodline' means it returns to the land, without which happening the Land's Choice won't have it to a worthy commoner later. When and if it comes back, it's not yours anymore, just like it wouldn't be if your PC died and came back to life and you kept playing them instead of switching to a blooded heir.
The game has no real win condition, but if your bloodline is lost this way, you have to start from scratch, as if you'd never played the game before. Even if you were #1 in the rankings when your character's line ended.

The rule talk about a fractured state with several great captains... I assume the player would need to be one of them... a distant cousin that was not known, an unrecognized bastard... etc.. The domain would be split up and the player would get part of it...

Our games are silly in that by turn 10 or 12 there will be many dozens of blooded scions available to become lieutenants. But today, there's NONE.

Yeah... though by turn 10-12 there will be many dozens of roads, buildings, and fortifications, and for now there's none, too.

I figured that anything needing an action to establish - an alliance, a vassalage, a state X, was off limits to start the game with. Of course I have histories and relationships with other realms ... but nothing with an action cost.
